Please bring your most current insurance card(s) to every visit, so we have the most current information.
Your insurance co-pay and PCP referral will be due upon check in. We do accept most major credit cards.

Office Hours
The office is open Monday-Thursday from 8:30am to 5:00pm and Friday 8:30am to 4:30pm, appointments can be made by calling (508) 473-4323, during office hours.
Emergencies
There is a neurologist on call 24 hours per day, 7 days a week. Our Neurologist can be reached for non-life-threatening emergencies by calling (508) 473-4323 and choosing option number 4. If you have a life-threatening emergency, you should dial 911 and go to your nearest hospital.
